Home & Community Care Program (H & CC)

Supporting Independence, Health, and Well-Being at Home

The Pinaymootang Home & Community Care (H&CC) Program provides compassionate, respectful, and culturally appropriate care to support community members in maintaining their health, independence, and quality of life while living safely at home.

 

Services Include

  • Home care nursing.

  • Client assessments.

  • Foot care services.

  • Home support services.

  • Personal care and in-home support.

  • Medical supplies and equipment.

 

How We Support Community Members

The H&CC Program helps individuals with:

  • Daily routines and personal care.

  • Home visits and wellness monitoring.

  • Exercise and mobility support.

  • Care coordination and referrals.

  • Access to mobility and safety equipment.

 

Mobility Equipment Support

Eligible clients may receive assistance accessing equipment such as:

  • Wheelchairs.

  • Walkers.

  • Canes.

  • Other approved mobility aids.

 

Who We Serve

Services are available to community members who require support due to:

  • Health conditions or home care needs.

  • Recovery after hospital discharge.

  • Disabilities affecting daily living.

  • Palliative and end-of-life care needs.

 

Accessing Services

Services begin with a referral and nursing assessment to determine eligibility and required supports.

Referrals can be made by:

  • Individuals requesting services.

  • Family members or caregivers.

  • Healthcare providers.

  • Community agencies and support programs.
