Pinaymootang’s Home & Community Care (H&CC) Program is dedicated to providing compassionate, respectful, and culturally appropriate nursing and personal care services. Our goal is to support community members in maintaining their health, independence, and quality of life while remaining in the comfort and safety of their own homes.

People recovering from an acute illness, injury, or managing a chronic condition who require ongoing care after discharge.

How to Access Services
Accessing Home & Community Care services begins with a referral and assessment process.

Referral Process

A nursing assessment is completed for all referrals to determine eligibility and identify the appropriate level of care and services required.
