Pinaymootang’s Home & Community Care (H&CC) Program is dedicated to providing compassionate, respectful, and culturally appropriate nursing and personal care services. Our goal is to support community members in maintaining their health, independence, and quality of life while remaining in the comfort and safety of their own homes.

 

Our Services

The H&CC Program offers a range of essential health and support services, including:

 

• Home Care Nursing

• Client Assessments

• Foot Care Services

• Home Support Services

• Medical Supplies and Equipment

• Personal Care and In-Home Support

How We Can Help

We provide support with everyday activities and services that promote independence and safety at home, including:

• Assistance with daily routines and personal care

• Home visits and wellness monitoring

• Exercise and mobility support

• Coordination of care and community resources

• Access to medical and mobility equipment

• Mobility & Safety Equipment

 

The Home & Community Care Program can assist eligible clients in obtaining equipment that enhances mobility, safety, and independence, such as:

 

• Wheelchairs

• Walkers

• Canes

• Mobility aids

• Other approved assistive devices

 

Who We Serve

The Home & Community Care Program provides services to community members who require support due to health, disability, or age-related needs, including:

• Individuals with Home Care Needs

• Community members of all ages and genders who have been assessed as requiring home care services.

• Individuals Transitioning Home from Hospital

• People recovering from an acute illness, injury, or managing a chronic condition who require ongoing care after discharge.

• Individuals Living with Disabilities

• Those who need assistance with activities of daily living and support to remain safely at home.

• Palliative and End-of-Life Care Clients

• Individuals living with a life-limiting illness, as well as their families and caregivers, who require care, comfort, and support.

 

How to Access Services

Accessing Home & Community Care services begins with a referral and assessment process.

 

• Referral Process

• A nursing assessment is completed for all referrals to determine eligibility and identify the appropriate level of care and services required.

 

Who Can Make a Referral?

Referrals can be made by:

 

• Individuals seeking services for themselves

• Family members or caregivers

• Health care providers

• Community agencies and support programs

• When appropriate, the Home & Community Care Program can also connect clients with additional community resources and specialized services.
